We’re here to care for your child—and keep things running smoothly for all families. Here’s what to know:
⏰ Need to Cancel?
Please let us know at least 2 hours in advance if you can’t make it. Missed appointments may incur a $30 fee.🚗 Running Late?
If you arrive more than 15 minutes late, we’ll do our best to fit you in—but on busy days, we may need to reschedule.🚨 Emergencies Happen

Before booking, check with your insurance to confirm coverage for well-child visits. Medicaid covers one physical per year.👀 Supervision Required
Parents must supervise children at all times while in the office.🧼 Help Us Stay Clean

To give each child the attention they deserve, we encourage you to call ahead for same-day appointments rather than walking in.
Medical Records & Transfers
We’re happy to help make transitions smooth and stress-free. Here’s what to know about accessing your child’s records:
📋 Immunization Records
Transferring to a new doctor? We’ll provide a copy of your child’s immunization record free of charge.📠 Chart Transfers
Just fill out an authorization form (at our office or your new provider’s). We’ll fax your child’s chart at no cost, usually within 2 business days.💾 Full Records Requests
Need a full chart? With a signed authorization form:
Printed copy – $1/page for the first 25 pages, then $0.25/page (per Florida law)
USB copy – $25 per child
Please note: The USB is not encrypted, so for privacy and security reasons, it cannot be mailed to another provider.

Financial Policy
We’re committed to providing excellent care and keeping billing simple. If you ever have questions about fees, we’re happy to help!
💰 Payment & Insurance
Payment is due at the time of your visit unless other arrangements have been made.
We accept cash, VISA, Mastercard, Discover, American Express, Apple Pay, and Google Pay.
We participate with many insurance plans and will gladly submit claims on your behalf.
If we are out-of-network with your insurance, we cannot bill your plan or provide claim forms. In most cases, the visit will be changed to self-pay, or we may not be able to see your child.
⚠️ Important for Medicaid Families: If we are not in-network with your Medicaid plan, we cannot see your child and cannot accept self-pay. This is a Medicaid regulation, and we appreciate your understanding.
📋 Understanding Your Coverage
Please review your insurance benefits before your visit.
Some services may not be covered, and payment is expected for those at the time of service.
Co-payments are required at check-in 💳
⚠️ Delinquent Accounts
Unpaid accounts may be referred to a collection agency.
If this happens, your child will no longer be able to be seen at our office.
You’ll also be responsible for any collection fees, attorney costs, or court fees associated with the process.
