Appointments

We’re here to care for your child—and keep things running smoothly for all families. Here’s what to know:

  1. ⏰ Need to Cancel?
    Please let us know at least 2 hours in advance if you can’t make it. Missed appointments may incur a $30 fee.

  2. 🚗 Running Late?
    If you arrive more than 15 minutes late, we’ll do our best to fit you in—but on busy days, we may need to reschedule.

  3. 🚨 Emergencies Happen
    We aim to keep wait times short, but urgent cases may take priority. Thanks for your patience and understanding.

  4. 📅 Annual Physicals
    Before booking, check with your insurance to confirm coverage for well-child visits. Medicaid covers one physical per year.

  5. 👀 Supervision Required
    Parents must supervise children at all times while in the office.

  6. 🧼 Help Us Stay Clean
    Please tidy up after your child and help us keep our space safe and welcoming. Damaging office property is not okay.

  7. 📞 We Prioritize Appointments
    To give each child the attention they deserve, we encourage you to call ahead for same-day appointments rather than walking in.

Medical Records & Transfers

We’re happy to help make transitions smooth and stress-free. Here’s what to know about accessing your child’s records:

  1. 📋 Immunization Records
    Transferring to a new doctor? We’ll provide a copy of your child’s immunization record free of charge.

  2. 📠 Chart Transfers
    Just fill out an authorization form (at our office or your new provider’s). We’ll fax your child’s chart at no cost, usually within 2 business days.

  3. 💾 Full Records Requests

    • Need a full chart? With a signed authorization form:

      • Printed copy – $1/page for the first 25 pages, then $0.25/page (per Florida law)

      • USB copy – $25 per child

        • Please note: The USB is not encrypted, so for privacy and security reasons, it cannot be mailed to another provider.

        • Please allow up to 5 business days for processing.

Financial Policy

We’re committed to providing excellent care and keeping billing simple. If you ever have questions about fees, we’re happy to help!

💰 Payment & Insurance

  • Payment is due at the time of your visit unless other arrangements have been made.

  • We accept cash, VISA, Mastercard, Discover, American Express, Apple Pay, and Google Pay.

  • We participate with many insurance plans and will gladly submit claims on your behalf.

  • If we are out-of-network with your insurance, we cannot bill your plan or provide claim forms. In most cases, the visit will be changed to self-pay, or we may not be able to see your child.

  • ⚠️ Important for Medicaid Families: If we are not in-network with your Medicaid plan, we cannot see your child and cannot accept self-pay. This is a Medicaid regulation, and we appreciate your understanding.

📋 Understanding Your Coverage

  • Please review your insurance benefits before your visit.

  • Some services may not be covered, and payment is expected for those at the time of service.

  • Co-payments are required at check-in 💳

⚠️ Delinquent Accounts

  • Unpaid accounts may be referred to a collection agency.

  • If this happens, your child will no longer be able to be seen at our office.

  • You’ll also be responsible for any collection fees, attorney costs, or court fees associated with the process.
